  • To achieve this, we need to maintain accurate records about you, your health, and the care we have provided or plan to provide to you.
  • We understand that you value your privacy and the security of the personal information held about you. Our guiding principle is that we hold your records in strict confidence.

Information We Record

As part of providing a professional, safe and efficient service, there is certain information that we must record. This includes details of drugs and appliances dispensed against NHS prescriptions, significant advice given, referrals made to other health professionals, and any other relevant information.

The information we record may include:

  • Basic personal details: your name, address, date of birth and next of kin.
  • Prescription history: records of medicines prescribed to you by your doctor or another qualified prescriber that have been supplied by this pharmacy.
  • Over-the-counter medicines: details of medicines purchased from the pharmacy without a prescription.
  • Clinical notes: other specific details and notes regarding your health and medical treatment.
  • Continuity of care: information relevant to your continued care from other people who care for you and know you well, such as relatives or external health professionals.
  • Additional services: documentation of any other services we provide to you, such as a flu vaccination.

How We Process Your Information

We process your personal data, including information related to prescriptions and any other pharmacy and healthcare services we provide to you (e.g. medicines reviews, vaccinations, stop smoking services). We process this data for three primary purposes:

  • Your care: providing pharmacy services and care to you. This includes receiving service and health records information if you have selected our pharmacy, and sharing your information with your GP and others in the wider NHS.
  • Our payments: sharing your information with the NHS Business Services Authority, others in the wider NHS, and occasionally local authorities. Limited information is shared with external parties who negotiate and check the accuracy of our payments.
  • Management: sharing limited information with the NHS Business Services Authority, NHS Digital, others in the wider NHS, and sometimes local authorities. Information is also shared with external bodies to ensure we maintain appropriate professional standards and that all service declarations are accurate.

Lawful basis for processing: we process your personal data in the performance of a task carried out in the public interest for the provision of healthcare and treatment. A registered pharmacist is strictly responsible for maintaining the confidentiality of your information.

Data retention: we hold your information securely for as long as advised by NHS protocols.

Managing Your NHS Health Records

  • Electronic prescriptions: we may ask whether you wish to choose our pharmacy to receive and process your future electronic prescriptions. If you nominate our pharmacy, you can change or cancel your selection at any time by speaking to a member of the pharmacy team.
  • Shared record consultation: our pharmacy professionals may consult relevant records to support your care, such as NHS Summary Care Records or local shared care records. We align strictly with NHS protocols for the use of these records. We may ask for your explicit permission to consult these records while you remain under our care, to ensure vital medical information is visible to our professionals.

Your Rights Regarding Your Data

You have the right to confidentiality under the UK General Data Protection Regulation (UK GDPR), the Data Protection Act 2018, and the common law duty of confidence.

We fully comply with the NHS Code of Practice on Confidential Information. Furthermore, our pharmacists are bound by professional standards to keep your records confidential, secure and accurate. All staff employment contracts contain strict confidentiality requirements.

Your Right to View Your Health Record

  • You have the right to request a copy of all pharmacy records held about you (generally in paper or electronic form).
  • There is no charge for a printed copy of the information we hold about you.
  • We are legally required to respond to your request within one month.
  • To protect your security, you must provide adequate identification (such as a passport, full driving licence, or credit/debit card) and sufficient information (full name, address, date of birth) before any data is released.

Rectification and Objection

  • If you believe any information we hold on you is inaccurate or incorrect, please inform us immediately so we can correct it.
  • You have the right to object to us holding your information.

National Data Opt-Out

You have the right to opt out of the NHS using your data for planning and research purposes. You can set or change your preferences by:

  • Visiting the online portal at nhs.uk/yournhsdatamatters.
  • Using the official NHS App.
  • Writing by post using the instructions provided on the NHS portal link above.
  • Calling the NHS Digital contact centre on 0300 303 5678 (open Monday to Friday, 9am–5pm).
